At 12:08 am Dane County Deputies, along with Middleton Fire and EMS, were dispatched to a head-on crash on USH 14 near Twin Valley Road, in the Town of Middleton.  Initial reports indicated a pickup truck was traveling westbound on USH 14 when it collided head-on with an eastbound semi-tractor and trailer.

Witness reports indicated the pickup truck, a 1998 GMC Sonoma, was traveling westbound in the eastbound lane of USH 14 when it collided with a 2003 Freightliner semi-tractor and trailer, traveling eastbound.  The driver of the Sonoma, Allen L. Henke, age 24, sustained significant injuries and had to be extricated by fire personnel.  Henke was transported to a Madison area hospital by Med-Flight.  The driver of the semi, Michael R. Perkins, age 40, sustained minor injuries and was transported by Middleton EMS to an area hospital for treatment.  Both Henke and Perkins were wearing seatbelts at the time of the crash.

Dane County Sheriff Crash Reconstruction personnel assisted the patrol deputies with investigating the crash.  Alcohol appears to be a contributing factor in the crash, with potential charges of Causing Injury by OWI and Operating Left of Center pending the completion of the investigation.
