The Dane County Division of Public Health and the Madison Department of Public Health are seeking nominations for their annual joint public health awards. The 8th annual Public Health Awards will be presented Monday, April 4 at 1:30 p.m. at Warner Park Community Center. “The great work of public health in our community involves many partnerships and collaborations,” said Dane County Executive Kathleen Falk. “These awards celebrate some of the unsung heroes in these efforts.” “Community partnerships are more vital than ever before," said Mayor Dave Cieslewicz. "We must ensure that we continue to work to protect public health - in traditional places and in new areas that would be difficult to reach without the team work of these vital community heroes.” Nominees should be individuals, businesses and organizations who enhance Madison and Dane County public health programs through advocacy, outreach, education and/or direct services. Nominees could be from the public and private sector, including volunteers, direct service providers, educators and policymakers. Environmental health, health care, health promotion and disease/injury prevention are examples of program areas in which nominees may be involved. More information on the awards, including an application form can be found on the internet at http://www.ci.madison.wi.us/health/commhealth/2005phwawards.htm Deadline for submitting nominations is February 18, 2005. # # #
